It is with pure excitement that I share this wonderful news! During the first week of 2018, Mission Bay Day Program submitted a grant proposal to the Department of Developmental Services (DDS) in order to request assistance with adherence to the new HCBS Final Rule regulations that must be implemented fully by March of 2022. Mission Bay recently received the fantastic news that some of the items that were requested in the grant, had been approved! One large part of this grant includes that a Person-Centered Profile and/or Person-Centered Behavior Plan will be developed for each person that attends Mission Bay Adult Day Program (service code 515.) What exactly does this mean? Over the next two years, participants will assist in the creation of a personalized plan to assist day program staff (and potentially other support members too) in providing support that enables each person to live a life that intentionally integrates what is important to, and for, the individual! The Person-Centered Plans will be completed by Yassamin Faraj, a Board Certified Behavior Analyst (BCBA) who founded Mosaic Behavior Consulting, and who has worked with people with developmental disabilities for more than 20 years. As part of the assessment process, members of each participant's support team will be requested to contribute to the person-centered plan process in order to share their perspective about the person's interests, hobbies, strengths, and how to best support the person overall.
