Mosaic Behavior Consulting is an approved consultant for San Andreas Regional Center, and works closely with residential care homes, group homes, specialized rate facilities, and the newest type of home--Enhanced Behavior Support Homes.
Services may include:
- Functional Assessment preparation
- Behavior Plan development
- Development of data collection forms and ongoing analysis of data
- Initial and ongoing training for staff
- Participation in meetings as a member of the interdisciplinary team
- Assistance with goal selection and planning
- Implementation and training on relevant evidence-based practices
- Focus on compliance with HCBS Final Rule & Person-Centered Planning
Yassamin Faraj has direct experience working in various roles as a previous service coordinator for adults with disabilities, a member of the Quality Assurance team, a special education teacher and teachers aide, and a behavioral specialist. Knowledge of regulatory agencies such as Title 17, Title 22, The Lanterman Act, and a keen interest in staying current on relevant topics and changes assist in not only providing quality care to the individual, but also in supporting the team in meeting regulatory requirements.
